in the presence of jaundice:

small GB ==> cholecystitis / impacted stone
(GB chronically inflamed, scarred)
large GB ==> NEOPLASM
(GB intrinsically normal, can dilate)
- pancreatic head
- cholangioCa (distal CBD)
- ampulla of Vater
- duodenal papilla
