- cirrhosis (alcoholic or postnecrotic)
- hemochromatosis
- hepatic toxins (aflatoxins)
- schistosomiasis
- glycogen storage disease type I (von Gierke)
- Thorotrast
- invades blood vessels in 75%
- a/w cirrhosis in 75%, usually post-necrotic
