etiology:
- thrombus (OCs, dehydration, septicemia, polycythemia vera,
paroxysmal nocturnal hemoglobinuria)
- congenital (web in hepatic v. or IVC)
- tumor or other mass (eg, hydatid cyst)
- sickle cell anemia
- trauma
